State suspends fines against Shenandoah facility

SHENANDOAH, Iowa - The state of Iowa has suspended $47,500 in fines against the Garden View Care Center in Shenandoah, following an investigation that found numerous violations, including inadequate staffing and resident abuse.

State inspectors substantiated four complaints, citing the nursing home for multiple violations of both federal and state regulations.

Issues included insufficient staffing, failure to report abuse, and repeated infection control violations dating back to 2020.

Despite these findings, the fines were suspended after the facility's director, Cindy Willis, reported management pressure to cut staffing further due to budget constraints.

However, the federal government could still impose penalties for the violations.

The nursing home has faced over $145,000 in federal fines in the past three years and holds the lowest possible ratings from the Centers for Medicare and Medicaid Services.
